Inclusions

The Bundled Rate plus Outlier Per Diems includes:

1. Those comprehensive facility, professional services, supplies, and equipment necessary to perform the Bundled Procedure and rendered by PROVIDER. 2. Consultation and treatment planning, radiation treatment planning and simulations, respiratory motion management, physics & dosimetry, radiation treatment management and consultation during the bundled period. 3. If combination therapy is prescribed and rendered for lung or prostate cancer (ie: 3D-CRT and IMRT), it will still be considered a single episode and will be billed using the greater of the case rates associated with each treatment. The Bundled period for the procedure begins on the date of treatment planning and lasts for 90 days, ending no sooner than 30 days following the last day of prescribed radiation.

Candidacy

Candidacy for the bundled procedure will be determined by the Provider.
