Hazelden Betty Ford Partial Hospitalization

The Bundled Rate plus Outlier Per Diems includes:

1. Clinical sessions are generally 5 hours a day, 5 days a week. Sessions are based on medical necessity and patient need, up to but not exceeding 24 treatment days. 2. Room & board is safe and supportive housing for patients engaged in Day Partial level of care. Room and board is offered on program and non-program days, including weekends.Lodging available based on need up to but not exceeding 35 days. 3. An initial screening will be completed prior to admission to ensure that the level of care can appropriately meet patient’s unique needs. The Bundled Rate covers outpatient services from the day of admission to the outpatient treatment with up to 24 outpatient day(s). Should the patient require outpatient services beyond the 24days, per diem payments will be applicable for each day of additional outpatient services related to the Bundled Procedure.

Candidacy for the bundled procedure will be determined by the Provider.
