Hazelden Betty Ford Intensive Outpatient Treatment

The Bundled Rate plus Outlier Per Diems includes:

1. Clinical sessions are generally 3 hours per day, groups meeting 3 to 4 times per week. Sessions are based on medical necessity and patient need, up to but not exceeding 24 treatment days. 2. An initial screening will be completed prior to admission to ensure that the level of care can appropriately meet patient’s unique needs. The Bundled Rate covers outpatient services from the day of admission to the outpatient treatment with up to 35 outpatient day(s). Should the patient require outpatient services beyond the 35days, per diem payments will be applicable for each day of additional outpatient services related to the Bundled Procedure.

Candidacy for the bundled procedure will be determined by the Provider.
