Inclusions

The Bundled Rate plus Outlier Per Diems includes:

1. Telemedicine Introduction Visit. 2. Sleep Profiler Study 3. Sleep Environment Assessment 4. Laboratory testing including the following tests only: Magnesium, Vitamin , B1, B12 and E, TSH, Iron Panel 5. Sleep Coaching (1 session) 6. Telemedicine Visit Summary, Written Summary Report and Treatment Recommendation 7. Clifton Strength Assessment 8. Home Sleep Apnea Study The Bundled Rate Period for the Covered Services begins the day of the procedure and ends after the PATIENT's same day discharge from the Bundled Procedure.

Exclusions

Prescription medications. Diagnostic studies related to sleep apnea except for the home sleep apnea study. Laboratory Testing except for the specific test listed above. Any testing not listed below as included. Virtual options are only available if the patient resides in Maryland, Texas, and Florida.

Candidacy

Candidacy for the bundled procedure will be determined by the Provider.
