Inclusions

The Bundled Rate plus Outlier Per Diems includes:

1. Remote, Peer to Peer review of medical records by an orthopedic provider specializing in member's diagnosis 2. Genetic risk screening and clinical trials screening 3. Written summary of findings and treatment options sent to referring physician, who has agreed to share with the patient 4. Patient must be outpatient at the time of the OSO. The patient must have a primary diagnosis. The diagnosis/case must be in a participating CHLA program. The patient must have a local, treating physician who will be the recipient of the report. The Bundled Rate Period for the Covered Services begins the day of the procedure and ends after the PATIENT's same day discharge from the Bundled Procedure.

Exclusions

ADDITIONAL DIAGNOSTIC SERVICES, PREOPERATIVE TESTING, CONSULTATION OR EVALUATION SERVICES ARE EXCLUDED FROM THE BUNDLED RATE.

Candidacy

Candidacy for the bundled procedure will be determined by the Provider.
